The Wakefield Questionnaire

The following depression screening quiz, known as The Wakefield Questionnaire, is presented here to help you become more familiar with the signs and symptoms of depression.

This questionnaire is not intended to replace a visit to a mental health professional. Only a professional can diagnose depression. The completed questionnaire can be shown to a healthcare professional to help open a dialogue about depression. This questionnaire contains groups of statements. Read each group of statements carefully, them click the radio button next to the selection which best completes each statement as it applies to you. Make sure you choose the statement that describes how you are feeling now, not how you were feeling or how you hope to feel in the future.
